Delivery Waiver

Deliveries are done curbside. If we are requested to unload the pallets any place else, a waiver must be signed.

Delivering beyond the curb may result in additional charges to accommodate the additional truck time.

"I have requested this procedure and have been advised of the risks for taking a truck or machine past the curb. This damage could include but is not limited to, the curb and gutter, sidewalk, driveway, grass, sprinkler systems, personal property, etc. and I agree to indemnify and hold harmless AgriScape LLC doing business as AgriScape Sod and its affiliates for any and all damage that may be done."

Follow-up needed: same as the Loading Waiver — this form needs a PHP handler wired up (matching the contact/quote form pattern) so submissions are actually recorded/emailed with a timestamp, giving you a real consent record like the current Squarespace form does.
